Job Description

We are seeking a Nurse Practitioner or Physician Assistant to provide high-quality, patient-centered care to residents in skilled nursing facilities throughout Mt. Pleasant, MI. This role offers clinical autonomy, flexible scheduling, and the opportunity to make a meaningful impact in post-acute and wound care.

Key Responsibilities

Conduct on-site rounds and comprehensive patient assessments.

Diagnose and manage acute and chronic conditions, including wound care needs.

Prescribe medications and develop individualized treatment plans.

Address acute and emergent clinical issues.

Communicate effectively with facility nursing staff via phone or secure messaging.

Maintain timely, accurate clinical documentation using a company-provided iPad.

Maintain reliable on-site attendance and comply with clinical, facility, and regulatory standards.

Schedule & Work Environment:

Part-time opportunity with flexible scheduling.

Facility shifts are typically 6 hours, with rounds generally beginning between 7:00–10:00 a.m.

No on-call or overnight requirements.

iPad with LTE provided for mobile charting.

Qualifications:

Active, unrestricted U.S. NP or PA license.

Board certification required: NP: ANCC, AANPCB, PNCB, or AACN. PA: NCCPA.

Clinical experience in post-acute care, skilled nursing, wound care, or a related setting preferred.

Qualified backgrounds include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine & Rehabilitation, and Geriatric Medicine.

Strong clinical judgment, communication skills, and commitment to patient-centered care.
